The Physiology of an Erection

Before understanding the medications, it is important to know what happens naturally during an erection.

An erection depends on proper nerve signals, blood flow, and muscle relaxation in the penis. When a man becomes sexually aroused:

  1. Nerves release nitric oxide (NO) into the erectile tissue (corpora cavernosa).

  2. Nitric oxide activates an enzyme called guanylate cyclase.

  3. This produces a chemical messenger called cyclic guanosine monophosphate (cGMP).

  5. As blood enters and is trapped inside the penis, it becomes firm.

After sexual activity, another enzyme—PDE5—breaks down cGMP, the muscles tighten again, and the erection subsides.

What Goes Wrong in Erectile Dysfunction

In men with ED, this natural process becomes disrupted. The common reasons include:

  • Insufficient nitric oxide release

  • Reduced blood flow due to cardiovascular disease or diabetes

  • More blood enters and stays inside the penis

It is important to note that these medicines do not create an erection on their own. Sexual arousal is still necessary to release nitric oxide and start the process.

Sildenafil was the first major PDE5 inhibitor. It has:

It usually starts working within 30 to 60 minutes. Some men feel effects sooner, but it’s best taken on an almost empty stomach for optimal absorption.

b) Duration

Its effects last about 4 to 6 hours, meaning it helps achieve an erection within that time window.

Tadalafil is not significantly affected by food, allowing flexibility in timing.

  • Allows more natural, spontaneous sexual activity

Because of its long action, some men use daily low-strength tadalafil, which maintains steady support for erectile function.
